The Vital Statistics Office provides birth
and death certificates for Florida only. The staff reviews certificates for
accuracy and completion and provides assistance with the preparation and
filing of vital records to funeral directors, medical examiners, and
hospital personnel. The Leon County Health Department's Vital Statistics
Office does not process Marriage Licenses, Divorce Records, Probate, Wills,
or Social Security Cards. All birth certificates for years not mentioned
below must be requested from the State Office of Vital Statistics in
Jacksonville, Florida (904-359-6900).

BIRTH CERTIFICATES
Birth certificates can be issued only to the
Registrant if of legal age (18 or older). Parents listed on the birth
record and legal guardians must provide guardianship papers. Legal
representatives of one of the above persons, or other individuals must
provide a recorded or certified copy of a court order.
