When calling EMS to request an ambulance, we ask
that you please have the following information available, in order for us to
better serve you: Location of the patient (room number), chief complaint, age,
present status, desired response mode, and any other pertinent info such as
vital signs and code status.
If a patient is being transferred from a long
term care facility, you must complete and fax a physician certification form to
(850)921-4100, and give the original to the responding crew. Calling 911
does not circumvent this requirement. If you you need a form, call
(850)921-0900 and we will be happy to fax you one. You can also download

This form must be completed in advance for any non-emergency transport from a
long term care facility.
